Job Summary:
Under the direction of the Project Director of the CBHS+ Programs, the Fatherhood Program PN will assist in the development and implementation of the Fatherhood program for CBHS+ and provide outreach, needs assessment and individual or group management to fathers/male partners of the pregnant women in the project’s target area. This position is located at the PCI’s US & Border Programs office. The position is full time.
Key Duties and Responsibilities:
- Conduct outreach to fathers/male partners of the pregnant women in the program, increasing their awareness and knowledge of health issues and accessing health services.
- Plans and implements health education sessions, specific events, support group activities, and learning projects including preparation, facilitation and follow-up relating to parenting skills (daily involvement, reading, relationship skills, health concerns, DV, possibly father-child activities).
- Prepares and disseminates educational information and material for a culturally-sensitive program design and conducts educational sessions and presentations.
- Provide individual or group case management and conduct home visits to fathers/male partner of program participants to address needs/concerns and evaluate their progress towards achievement of project objectives.
- Identify clients in need of supplementary services and make referrals to appropriate community and health resources in San Diego and serves as a point of contact for coordination of services.
- Develop and maintain a well-stocked health and empowerment information/education “Library” related to father/male partner involvement.
- Collect and maintain relevant project data per grant/contract specifications.
- Participate in agency and project meetings.
- (repeat of above)Maintain continuing education as appropriate and assist with special projects as assigned by Supervisor.
- Serve as Fatherhood program liaison to all program staff and the community.
- Perform all other duties as deemed necessary by the Sr. Program Manager or Project Director
Special Requirements
- Must have reliable transportation
- Must be available to work a flexible schedule to meet with families some evenings and weekends.
Required Qualifications:
- High School diploma or GED certificate required with a minimum of 2 years professional experience in health, psychology, or social work field. CHW certification a plus.
- Experience in community work involving fathers or males and/or Parents as Teachers training or experience a plus.
- Knowledge of US health care systems, mental health services and exposure to binational or international collaborative programs a plus.
- Fluent in Spanish and English (read, write, speak) required and knowledge about bicultural issues related to Latino clientele.
- Proficiency in MSOffice: Word, Excel required.
- Able to work independently and as a team-player with creativity in problem solving.
- Detail-oriented with strong communication, presentation, and interpersonal skills.
- Reliable transportation required. Must be available for occasional local travel (within California and US). Availability to accept some weekend and evening assignments.
